Withdrawal Request Form

We're sorry to see you go! Whether you're taking a break, changing direction, or just need to step away for a bit, we want to make the process simple and clear.

To officially request the cancellation of any service, including ongoing classes, birthday party bookings, competition team enrollment, or special event registrations, please complete the withdrawal form.

Class Withdrawal Policy:

To cancel a membership or drop a class, you must submit an official Withdrawal Form. This is the only accepted method of cancellation.


  • This form must be received on or before the 15th of the month to make that month your final month of service. (Ex: Submit by May 15th for a May 31st final day). Submissions on or after the 16th will result in one final payment and enrollment through the following month.

  • Once payment has been received, you are committed to that month. We do not provide partial refunds for withdrawing earlier than the end of a month.

  • Make-up tokens from previous absences are forfeited when you withdraw and will not carry over to re-enrollment. 

  • Evaluated skills are kept on file for one year from withdrawal date so ninjas can re-enroll where they left off.
